Do all states allow massage therapy to be billed to insurance?

Most can, some can't. Most insurance policies allow it, but sometimes it comes down to the individual's policy. There are a few things you'll need to get lined up before you send in your first bill also, which I cover in the course. Check with your state's licensing board for verification, and your client's specific policy.
